100% outpatient role --- Doctors are facing maddening insurance protocols and spending hours on the phone getting authorizing. Mountains of paperwork are increasing and reimbursement is steadily decreasing causing doctors to need to see more patients just to keep doors open. Let's go back to the old fashion way when doctors had time to listen and patients were happy with their care and the relationships with their family doctor. The PACE model involves keeping as well as maintaining the health and well being of a wonderful group of elderly participants within a team environment of providers and care givers. Having just a few clinic visits per day insures the chance to spend needed quality time with each person to help formulate and execute their own specific life plan. (Life plans are individuals plan of care to address specific needs.) PACE physicians work closely with PACE center teams of professionals to help insure that all participants have the opportunity to enjoy life to the fullest, while still residing in their own homes. Staying independent and continuing to live safely at home is important for seniors. PACE helps them achieve that goal. PACE's comprehensive care includes medical and nursing care, physical therapy, occupational therapy, nutritional services as well as social work support. PACE also offers day program that allows participants to socialize and join in activities. As it has become more and more difficult to deliver thorough individual medical care, PACE allows physicians the resources to again offer dedicated timely and meticulous patient care in a relaxed setting that caters to the individual . This personalized old fashion approach to medicine is not only appreciated, it is necessary as well.
You will provide care for 4 to 6 seniors daily. These seniors want an alternative to long term care placement. Allow seniors to live with dignity in their communities. PACE participants can remain in the secure surroundings of their own homes and communities while providing family members and care givers with much needed professional guidance, physical relief and emotional support. PACE is a place to go to meet new friends, participate in fun activities and receive your medical, social and emotional care. While most care is delivered in the adult day health center during the day, care is coordinated 24 hours a day. PACE's day-to-day monitoring of participants results in fewer hospitalizations, less reliance on prescription medications, better quality of life and better overall health.
