The Social Worker oversees the residents in Assisted Living by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs. Services will be provided in accordance with the United Methodist Communities Social Services Policy and Procedure Manual. The Social Worker is also responsible for fostering a climate that provides residents with the highest practical level of physical, mental and psychosocial wellbeing and quality of life.
Some duties include:
Develops, maintains and utilizes a listing of current community resources.
Advocates for the residents and their families.
Understands and meets all requirements for social service documentation, i.e. interdisciplinary care plan; d/c notes; advanced directives; resident rights, etc.
Prevents and addresses resident abuse.
Educates associates regarding residents’ rights including how to recognize and prevent abuse, neglect and mistreatment, resident dignity, independence and quality of life - as outlined in the Resident Rights.
