We are looking for a Care Transition Nurse Manager to join our team. This managerworks with patients, families, and a multidisciplinary primary care team to manage transitions of care that optimize health outcomes for patients with a recent hospitalization or emergency department visit. They educate patients on appropriate use of the emergency department, the importance of medication adherence, and self-care during acute illnesses.They work closely with Managed Care organizations, and they supervise a small team of Case Managers. They will spend 80% of their time in active clinical care management, and 20% of their time performing administrative/supervisory responsibilities.
This is a full-time position located on our main campus in South Omaha (30th & L.) The schedule will be 40 hours per week; Monday-Friday, 7am-5pm.

- Current RN or LPN license required.
- Two or more years of clinical nursing experience preferred.
- Prior experience in Case Management or Care Management required. - Two or more years of supervisory or management experience preferred.
- Basic Life Support (BLS) certification preferred.
- Proficiency in English required; proficiency in Spanish preferred.
- Strong computer skills, particularly in Microsoft Office products, required.
- Prior experience with an Electronic Health Record required. - Strong data analytical skills required.
