The mission of New Moms is to express the love of God by surrounding young moms and their children with all they need to transform their lives. New Moms is a 33 year old nonprofit organization, with a budget of about $3 million and a staff of about 40, focused on serving young moms in Chicago and the near western suburbs. New Moms’ model provides a comprehensive range of services to young moms and their children including supportive transitional housing, workforce development, early childhood services, and parenting support and education. New Moms’ headquarters is located on the west side of Chicago in the Austin neighborhood. Our social enterprise, Bright Endeavors, which provides paid transitional jobs for young moms, is located nearby in the West Garfield Park neighborhood. We also have an office in Oak Park serving the near western suburbs.

About the Job:
This unique position is responsible for providing comprehensive, individualized case management to CPS families who identify as Students in Transitional Living Situations (STLS) and enrolled in the new Families in Transition Program (FIT). This seasoned professional will collaborate with the Housing Locator to move families into permanent housing and provide case management support for 15 families for up to 18 months enrolled at Howe Elementary School. Case management will focus on all family members to help them achieve their goals in the areas of education, employment, and life skills that support the retention of permanent housing and increase positive family outcomes. This position is located at our main office in Austin and spends the majority of their work time in the community. Salary $38-40k DOE.
Maintains a case load of up to 15 participants (and their children) who reside in the community.
Through a strengths-based approach, conducts thorough assessments of families, utilizing prescribed assessment tools on established schedule; develops written service plans, with quarterly updates at a minimum which includes steps to for increased education and/or unsubsidized employment and stabilization of housing at exit.
Conducts at least bi-monthly home-based visits with the participant that consist of coaching to enhance the parent-child relationship as well as service planning, life skills, and emotional support through a trauma-informed approach.
Performs crisis prevention as needed.
Works to engage participants in the program, working to maximize participant retention and minimize early exits as well as program safety.
Performs written and electronic recordkeeping functions as required for both internal and external databases on a timely basis.
Collaborates with the Housing Locator, community landlords, school partners and other community partners in the project that focuses on housing stabilization and eviction prevention.
Works in tandem with other New Moms staff to integrate services across the Agency.
Actively participates in weekly supervision, team meetings, and agency staff meetings.
Regularly participates in agency or professional group seminars or conferences to enhance professional knowledge and job skills.
Participates in an on-call schedule at a minimum for 1 week a month.

The successful candidate will posses:
Commitment to and passion for the mission of New Moms
Master’s Degree in the area of social work, counseling, public health or psychology preferred or Bachelors with 5+ years experience required.
2 years of professional experience in housing and homeless services preferred
Minimum 3 years of case management experience required
Endorses a “Housing First” philosophy which focuses on a Family-Centered, Trauma-Informed, Strengths-Based practice model that integrates harm reduction, motivational interviewing and positive youth development practices in working with homeless families required
High attention to detail and results-oriented approach to the work
Sound professional judgment and analytical ability, as well as the desire to work in a socially- and psychologically-pressured environment
Experience in the use of personal computers and Microsoft software applications
Excellent communication skills, both oral and in writing
Ability to manage multiple responsibilities, projects, and deadlines concurrently
Dynamic relationship-building skills to create and sustain fruitful relationships internally and externally
Ability to work occasional evenings
Ability to be on-call after hours and on weekends for emergencies
Must have access to a car and valid driver’s license
